Waiver
In consideration of being allowed to enter Bananas! Drop-In Jump & Play, use the equipment and facilities, and/or participate in any party, function, or program at Bananas! Drop-In Jump & Play, the undersigned on his/her own behalf and on behalf of the minor child(ren) identified below acknowledges, appreciates, and agrees to the following conditions: 

1.             I represent that I am the parent and/or legal guardian of the child(ren) identified below as participants, or in the alternative, I represent that I have obtained the express permission of the parent(s) and/or legal guardian of the child(ren) identified below as participants to execute this Waiver.

2.             I understand that Bananas! Drop-In Jump and Play is owned and managed by Bananas! Drop-In Jump and Play, Inc., an Ohio Corporation.

3.             I agree that the participants listed below and I shall comply with the terms set forth in this document as well as the posted “Rules of the Game.”  I agree that the participants shall only use the equipment and attractions only as directed.

4.             I agree that the participants listed below and I shall comply with all posted safety signs and the verbal instructions of the Bananas! Drop-In Jump and Play staff.

5.             I am aware that there are inherent risks associated with participation in the Bananas! Drop-In Jump and Play attractions, facilities, equipment, programs, and parties, including the risk of serious personal injury.  I, on my behalf and on behalf of the participants listed below, freely, voluntarily, and knowingly assume all such risk, both known and unknown, including, but not limited to, those risks that may arise out of the negligence of other children and adults using the attractions and equipment.

6.             I agree to immediately notify the staff of Bananas! Drop-In Jump and Play of any hazard observed during participation in the use of the attractions and equipment.

7.             I, for myself and the participant(s) listed below, our respective heirs, assigns, and personal representatives, hereby release and hold harmless,  Bananas! Drop-In Jump and Play, Inc., its affiliates, officers, members, agents, employees, stockholders, and directors from and against any claims, injuries, liabilities, and/or damages arising out of, or related to, our participation and/or use of the facilities, equipment, attractions, and/or events.
